NEPA Community Health Care is privileged to collaborate with area health and human service organizations through the Rural Health Partnership, facilitated by Endless Mountains Health System. Participants across Susquehanna County collaborate with one another to enhance programming for all people living and working in our communities.
Not least of the outcomes produced by this group is the fact that local service organizations are learning how to support one another and grow together in their combined mission to improve the health and wellness of Susquehanna County! Among the participating organizations are Barnes-Kasson County Hospital, Gracious Living Estates, Susquehanna County Interfaith, Meadow View Healthcare & Rehabilitation Center and Trehab, as well as consistent representation of the Susquehanna County Commissioners.
