Licensed Clinical Social Worker

The LCSW provides social work interventions to remove psychosocial barriers to health and wellness; collaborates with primary healthcare team, the patient/family and community liaisons to assure patients’ psychosocial and socioeconomics needs are addressed wherever possible; participates in monthly staff meetings and physical /behavioral health integration efforts; provides supervision to lesser credentialed social workers; and may participate in regional human services collaboratives as health center representative with a focus on improvement of quality of life in the health center service area.

Family Nurse Practitioner

The CRNP will function in the capacity of Family Nurse Practitioner in the care of patients across the lifespan in accordance with the established scope of practice as specified in the job description, by the Pennsylvania Board of Nursing, and by the certifying organization.

Clinical Coordinator | RN

The Clinical Coordinator plans, directs and coordinates primary care services, inclusive of medical and behavioral health, while supervising support staff and maintaining compliance with FQHC, Patient-Centered Medical Home and Meaningful Use regulations. The Clinical Coordinator facilitates compliance measures, carries out management responsibilities in accordance with organizational policy and applicable state and federal law, and develops policy and procedures necessary to meet a standard of excellence in all lines of patient care. The Clinical Coordinator actively participates in patient care and possesses strong primary care nursing skills.
