The LPC provides therapeutic interventions to remove psychosocial barriers to health and wellness; collaborates with the primary healthcare team, the patient/family and community liaisons to assure patients' psychosocial and socioeconomics needs are addressed wherever possible; participates in monthly staff meetings and physical/behavioral health integration efforts; and may participate in regional human services collaboratives as health center representative with a focus on improvement of quality of life in the health center service area.
Please note: This position works directly with students in the Susquehanna Community & Elk Lake School Districts throughout the academic year. During summer months, this position is housed within NEPA CHC health centers.
Nurse Manager (RN)
Under the supervision of the Director of Quality Assurance, the Nurse Manager is responsible for the effective coordination of the clinical support and reception staff across health centers, ensuring compliance with established policies and procedures. This role serves as a liaison between the administrative and clinical teams and functions as an adviser to leadership on matters pertaining to the operations of medical and behavioral health services. The Nurse Manager will participate in the active delivery of care as scheduling demands require and serves as a role model in attitude, attendance, behavior, professional appearance and in communication and interactions with other employees and patients. S/he demonstrates confidence in decision making, flexibility and responsiveness to changing demands, as well as the ability to prioritize to meet operational and other needs.
Please note: Weekly travel between health centers (Greenfield Twp, Hallstead, Montrose and Susquehanna) is required.
